This helps us get a better understanding of the impairments that might be causing your problem. We will then assemble a treatment plan for you. 

You can help the process along by sitting down before your first appointment and writing down the important facts about the history of your problem. By writing things down, you are more likely to include important facts and remember specific things we should know. Try to include the following information:
